Future 草榴社区 Students
Students with disabilities who apply to 草榴社区 are not required to identify the existence of a disability during the admissions process. However, documentation is typically required to register with Disability Services to receive appropriate accommodations. If you have questions, concerns, or delays regarding documentation, please do not let that discourage you from speaking with us to initiate the process and discuss options for accommodations.
While it is possible to register with our department at any time during one鈥檚 college experience, it is helpful to connect with Disability Services as early as possible to review your documentation, discuss your needs and resources, and ensure that your accommodations are in place in advance and ensure a good start to your 草榴社区 experience.
Please contact our office directly or inform the Admissions office that you are interested in meeting with us when setting up your college visit.
Submitting Documentation
A student鈥檚 documentation (most recent MFE, IEP, etc., verification of previous accommodations at another college/university, 草榴社区鈥檚 Disability Services Registration Form, etc.) can be submitted in person or via mail, email, or fax to:

Disability Services will contact students via their 草榴社区 email account if activated, the provided email account, or via phone within 5 business days upon receipt of documentation, unless extenuating circumstances arise.
If a student does not yet have access to their documentation, they are encouraged to move forward with contacting Disability Services to discuss their needs for support.

Documentation Guidelines
Documentation should demonstrate that a student has a disability as described in Section 504 of the Rehabilitation Act of 1973 and Title II of the Americans with Disabilities Act (ADA) of 1990. A disability is defined by the ADA as a physical or mental impairment that substantially limits one or more major life activities.
Documentation may consist of the following:
1. A full evaluation or diagnostic report, such as an ETR or MFE, to provide the most comprehensive information about the student鈥檚 needs.
2. A letter on letterhead including a qualifying professional鈥檚 signature, a diagnostic statement, the date of the diagnosis, the severity of the diagnosis, the medication or treatment plan, and recommended accommodations.
3. Documentation that demonstrates prior accommodations and proof of the diagnosis. This may include an IEP, a 504 plan, or verification from a previously attended university or college.
4. The Disability Services Registration Form.
Documentation Considerations
1. A qualified healthcare professional should complete the documentation.
2. A diagnostic statement identifying the disability should be included.
3. A description of current and past accommodations/services may be helpful.
